Why Review Is Needed
REM: State the consequences: rights restriction, financial cost, residence/care control, medical decision-making, and public confidence in procedure.
Safe phrasing: “The record should be reviewed to determine whether material allegations were verified before they were relied upon.” This is stronger and safer than stating that every actor acted in bad faith.
